There are two options to submit your story to SAWA Global Foundation.

Option One: download a Word document and save it to your computer, complete the required information and email the form.

Option Two: complete the PDF form online, print the document and mail or fax the form. Note that any information on the PDF form cannot be saved.

Visual materials such as video clips and photos can either be sent via email (small MB size only) or mailed to the address below.
